Crescent Post Acute Consultants (CPACS) is a telemedicine service designed specifically for Skilled Nursing Facilities in California. The platform connects skilled nursing facilities with physicians from multiple medical specialties to help manage patient conditions without the need for off-site visits. The service is offered at no cost to the facility and reduces delays related to scheduling and transportation.
CPACS offers access to a broad range of medical services, available as needed, without cost to the facility. It functions as a mini hospital, accessible within the facility setting.
Additionally, primary attendings receive free access to CPACS' EMR to view consultant notes and electronically document their H&P and routine progress notes.
